Enrollment Agreement

Our training Course will provide the knowledge and skill to work and as a phlebotomist working in the medical field. To successfully complete the phlebotomy program, every student will be required to perform the tasks and functions in which they are Learning. By the end of the 15 hour course a student must successfully complete 25 venipunture. This course will teach you all of the skills for the field of phlebotomy. Our instructors will guide you as you learn a variety of types of venipuncture, safety standards, basic anatomy and physiology, and other factors involved in this career. Topics you will learn HIPAA, OSHA, Patient Rights, Venipuncture with Vacutainers, Butterfly’s, Order of draw, Professionalism, Special Procedures, Patient Reactions, Hard blood draws, CLSI Standards. At our program you will attend a 5-day Mon-Fri 2:00pm-5:00pm or 6:00pm-9:00pm hands on phlebotomy training course a total of 15 hours in class followed by a short homework assignment given to the student in class by the instructor. This course meets national level phlebotomy certification status with the National Phlebotomy Certification Testing (NPCT)

You can take the tests up to three times with no extra charge. When the course is complete a certificate of completion will be provided.

Cancellation / Refund Policy: Each student will have 3 day cooling off period in which the student that Enrolls will have three business days to receive a full refund. A 3 day cooling-off period, commencing with the day an enrollment agreement with the applicant is signed or an initial deposit or payment toward tuition and fees of the institution is made until midnight of the 3 day following such date or from the date that the student first visits the institution, whichever is later. Evidence of personal appearance at the institution or deposit of a written statement of withdrawal for delivery by email or other means shall be deemed as meeting the terms of the cooling-off period. A minimum of $100 dollar deposit must be paid in order to hold your seat in the class. You may pay the full fee of $475 on the website but you will be subject to the three business day refund policy. The remaining balance must be payed in full the first day of class. Once the student pays for the remaining balance in class and has completed the first day there is no refund. Any cancellations without requesting to change to other dates will be subject to three business day cancellation policy stated above. A student will have a one time only to change there dates for any reason without paying another deposit. If the student does request a change of dates He/She will not be charged another deposit. Once the student changes there dates they will need to keep those dates or they will need to register again and pay another deposit of $100.

You will not receive your certificate of training until the course is paid for in full.

At Oquirrh Mountain Phlebotomy School LLC We do require that all students who enroll in the phlebotomy course possess a high school diploma, General Education Development (GED) Certificate and be at least 18 years old to take the course.

We do not acknowledge credit to students for other programs/courses taken. Students records and fees and registration will be stored for a 10 year period.

100% Attendance is mandatory for all students. A student can make up a class no charge on the dates posted on the website.

A student s behavior is very important in taking this class if for any reason a student is violent towards another student or the instructor or disruptive in any way and has had three warnings they will removed permanently from the program/course without refund or there certificate of completion.

We do not offer Job placement. However we provide a list potential employers to apply to on this website. We do not guarantee hourly wage levels for this type of training.
